PRIMARY CARE
Cash:
Thirty days of in-patient residential Primary Care Treatment
Insurance:
Thirty days of in-patient residential primary care Based on coverage
(If insurance does not pay the undersigned is responsible for full payment of services rendered.)
Services included in price for primary care treatment:
- Onsite detoxification protocol, if necessary
- Initial drug screen
- Basic lab work
- One visit with Medical Doctor
- One visit per week with Psychiatrist
- Family Program
- Treatment
Services not included in price for primary care treatment:
- Medications, other than detoxification protocol
- Additional drug screens
- Additional lab work
- Additional visit with Medical Doctor, if needed
- Therapy with consulting Therapist.
- Additional Marriage and Family Therapy
- Testing, MMPI ll, Millon lll, BSI
- Off site detoxification, if necessary

Pricing for Services not included are available upon request.
For those patients leaving Jolimar Recovery Center prior to completion of the first 30-days, an admission fee and physician’s fee will be charged along with a per day rate. All refunds will be made five (5) working days after the end of the month of discharge.
EXTENDED CARE
Cash
Thirty days of residential Extended Care treatment
While in extended care all services are the responsibility of the undersigned.
For those patients leaving Jolimar Recovery Center prior to completion of the thirty days of extended care no monies will be refunded.
OUTPATIENT TREATMENT
Primary Care Day Treatment
-Five Days a week 6 hours per day (20 days) 120 hours
Intensive Outpatient Treatment
-2nd Offense DUI - Three days a week (12 weeks) 108 hours
Outpatient Treatment(No Certificate)
-Three days a week for one month (12 days) 36 hours
Extended Care Day Treatment
- Five days a week for one month (20 days) 60 hours
-Three days a month for one month(12 days) 36 hours
